Forms are required each year ONLY for students requiring alternate transportation for childcare/special accommodations. Regular home to school transportation does NOT require a new form annually.

Please be sure to complete all the information clearly on your request form.

PLEASE NOTE - requests take approximately 48 hours or longer to process during the first week of school.  

We can NOT process requests, which are photos of completed forms.
